No reason to set every post to admin only. Just set new user registrations to admin activation.
Then for the new users, you'll have to manually check their IP against the spam lists.
... or just implement an external code verification which can't possibly be answered by anything but a human.
Go try to register on my forums and you'll see what I'm talking about. That one added step has completely eliminated spam on my forum, and has worked for 2 years. Captcha updates will always be a very temporary fix until the bots learn to break the new captcha. The fix that I implemented doesn't rely on any pre-formed code or image interpretation. Only a human can come up with the response to an actual question. (for now anyway)

I have seen this method used on other Forums which seem to be spam free. Don't think could be too hard to implement, but what do I know? Nothing.

yeah just logged in and saw this. still need to run a bunch of existing accounts against the Spammers IP list like Orkan was mentioning... think we have a handful that snuck in before I did the admin crack down...
